Diagnostic evaluation of dementia in the secondary health care sector

Phung TKT et al. – Adherence to clinical guidelines for dementia work-up is inadequate in the secondary health care sector. These findings call for improvement in organization of clinical dementia care, for education of specialists, and for changes in attitude towards making a diagnosis of dementia.
Methods- Nationwide registry-based study of quality of diagnostic evaluation for dementia in secondary health care sector
- Randomization of 200 pts from pt population (4,682 pts) registered for first-time dementia diagnosis in nationwide hospital registries during last 6 mo of 2003
- Medical record review for work-up completeness on which dementia diagnosis was based
- Use of evidence-based dementia guidelines as reference standards
- Satisfactory or acceptable completion of basic dementia work-up documented in 51.3% of pts
- Follow-up investigations for only 11.5% of pts with unsatisfactory work-up
- Dementia syndrome confirmed in 88.5% of cases
- Correct subtypes diagnosed in only 35.1%
