• Long-term oral antiplatelet therapy targeting specific platelet activation pathways: significant long-term benefit
• Antithrombin use: limited to acute setting
• Long-term dual oral antiplatelet therapy with aspirin and clopidogrel: proven efficacy but residual, considerable morbidity and mortality
• These effects may be partly due to incomplete inhibition of platelet activation and/or lack of long-term anticoagulant therapy
• Improvements in outcomes may be achieved by developing agents to inhibit other platelet activation pathways or by adding new anticoagulants prolonged duration after acute event
