Anterior and inferior to the umbo, the tympanic membrane meets the wall of the external bony ear canal at an acute angle. The angle can pose a surgical challenge, and the space it forms can harbor iatrogenic cholesteatoma, especially when the angle is quite sharp. Some otologists have wondered whether these angles are more acute–and hence more difficult to work with–in patients with otitis media.
