Richter GT et al. - Brain abscesses are now an infrequently reported complication of otitis media in the United States; when they do occur, they usually result from chronic otitis media with cholesteatoma.1-3 Patients often present with a prolonged history of otologic complaints including hearing loss, otorrhea, and otalgia. Symptoms suggestive of intracranial extension include fever, headaches, disequilibrium, and mental status changes.4Although most patients with suppurative otitis in the United States are diagnosed and treated before intracranial complications develop, antibiotics may mask the symptoms of otogenic complications and allow progression before diagnosis can be made.
