Lennon R et al. - Rosiglitazone has a direct and protective effect on glucose uptake in wild-type human podocytes. This represents a novel mechanism by which PPAR-gamma agonists may improve podocyte function in diabetic nephropathy.

The podocyte is increasingly being seen as central to the pathogenesis of diabetic nephropathy. Insulin sensitivity in glomerular podocytes is a novel and important finding in the context of understanding the pathogenesis of diabetic nephropathy. A key question is whether this cellular insulin sensitivity is lost in type II diabetes, as is seen in other insulin sensitive tissues, and therefore whether drugs can improve this sensitivity. This study for the first time demonstrates that podocytes can become insulin resistant, and that the PPARgamma class of drugs counteracts the effect, and in themselves improve podocyte insulin sensitivity.
