Effect of the site of measurement of waist circumference on the prevalence of the metabolic syndrome

Mason C et al. – Prevalence of the metabolic syndrome (MS) is modestly influenced by the anatomic site of waist circumference (WC) measurement. Efforts should be made to standardize the protocol for measurement of WC given its potential to influence research findings and clinical decision-making.
Methods- Study of whether WC is differentially associated with cardiometabolic risk factors based on its anatomic measurement site
- Quantification of the impact of measurement site on prevalence of MS
- Sample of 520 community-dwelling adults (208 men, 312 women)
- Measurement of WC at 4 common sites: superior border of iliac crest, midpoint between iliac crest and lowest rib, umbilicus, and minimal waist
- Measurement of blood pressure at rest
- Analysis of fasting blood samples for concentrations of high-density lipoprotein cholesterol, triglycerides, and glucose
- MS diagnosed by criteria of American Heart Association/National Heart, Lung, and Blood Institute
- Similar overall patterns of association between WC and cardiometabolic risk factors across anatomic locations of measurement
- In men, same prevalence of MS for WC measured at iliac crest, midpoint, umbilicus (21.2%) but lower (18.3%) using minimal waist
- In women, MS prevalences: 15.1%, 14.4%, 14.1%, and 13.1% using umbilicus, iliac crest, midpoint, and minimal waist, respectively
