Endocrine Treatment of Transsexual Persons: An Endocrine Society Clinical Practice Guideline

Hembree WC et al. - Transsexual persons seeking to develop the physical characteristics of the desired gender require a safe and effective hormone regimen that will suppress endogenous hormone secretion determined by the person's genetic/biologic sex and maintain sex hormone levels within the normal range for the person's gender. The authors recommend treating transsexual adolescents (Tanner stage 2) with suppression of puberty with GnRH analogues until age 16 years, after which cross-sex hormones may be given. They suggest suppression of endogenous sex hormones, maintaining physiologic levels of gender-appropriate sex hormones and surveillance for known risks and complications in adult transsexual persons.

| As the chair of this guideline's writing task force, I must inform the reader that the version of the guideline posted on June 22 to MDLinx is in draft form. The Endocrine Society is currently finalizing this guideline and the final version will be published in the September issue of the Journal of Clinical Endocrinology & Metabolism. One of the changes that will be included in the September published version is the change to the dosage in Table 12. The correct dosage of Parenteral: estradiol in MTF transsexual persons is 5-20 mg im every 2 wk and the correct dosage for Spironolactone is 100-200 mg/d.
