Adami S et al. – This study demonstrates that optimal vitamin D repletion seems to be necessary to maximize the response to anti-resorbers in terms of both bone mineral density (BMD) changes and anti-fracture efficacy.

Methods
  • An investigation to monitor if vit. D repletion in the treatment of osteoporosis could maximize the response to therapy
  • 1515 women with postmenopausal osteoporosis under treatment with anti-resorbing agents (alendronate, risedronate, raloxifene) for 13.1 mos with an adherence >75% were studied
  • The pts were classified as vit. D deficient or vit. D repleted according to risk factors or the level of 25(OH) vit. D above or below 50 nmol/l

Results
  • Vit. D deficient and vit. D repleted subjects differed significantly for annualized spine and hip BMD changes
  • 151 pts suffered from a new incident clinical fracture
  • The adjusted odds ratio for incident fractures in vit. D deficient vs vit. D repleted women was 1.77
