Lehinant SA et al. – There are new concepts for the treatment of acute decompensated heart failure. Several novel drugs act by modifying guanylate cyclase. Nesiritide and ularitide are natriuretic peptides acting on particulate guanylate cyclase. Cinaciguat is a novel soluble guanylate cyclase activator. Cinaciguat has a promising and novel mode of action: it activates cGMP synthesis by targeting sGC in its nitric oxide–insensitive, oxidized ferric or heme–free state. Preliminary studies of cinaciguat in patients with acute decompensated heart failure show substantial improvements in hemodynamics and symptoms, while maintaining renal function.
