Central post-stroke pain: Clinical characteristics, pathophysiology, and management
The Lancet Neurology, 08/12/09
Klit H et al. – A review of central post-stroke pain (CPSP) focuses on clinical characteristics, pathophysiology, and management. Future prospective studies with clear diagnostic criteria are essential for interpretation of epidemiological data.
Methods- Review of clinical characteristics, pathophysiology, and management for CPSP
- CPSP: neuropathic pain syndrome after cerebrovascular accident
- Characterization by pain and sensory abnormalities in body parts corresponding to brain territory injured by cerebrovascular lesion
- Presence of sensory loss and signs of hypersensitivity in painful area in CPSP pts may indicate dual combination of deafferentation and subsequent development of neuronal hyperexcitability
- Prevalence of CPSP unknown, partly due to difficulty in distinction from other post-stroke pain types
- CPSP treatment: difficult but most effective approaches target increased neuronal hyperexcitability
