Prevalence and prognostic significance of transient, persistent, and new-onset anemia after acute myocardial infarction

Hasin T et al. – Persistent or new-onset anemia occurs in a significant proportion of pts after acute myocardial infarction (AMI). Although resolution of anemia after AMI is associated with better outcomes, persistent or new-onset anemia portends increased risk for heart failure and death.
Methods- Study of 1,065 pts with AMIs with hemoglobin levels available postdischarge
- Pt categorization by anemia status at hospital discharge and follow-up: 1) no anemia, 2) resolved anemia, 3) persistent anemia, 4) new-onset anemia
- Primary endpoint: mortality and hospitalization for heart failure
- Cox models to evaluated association between anemia and primary endpoint
- Reference group: pts without anemia at the 2 time points
- At hospital discharge, anemia in 370 pts (34.7%)
- At follow-up, anemia resolved in 162 pts (15.2%), persistent anemia in 208 (19.5%), new-onset anemia in 55 (5.2%)
- During median 27-mo follow-up, 110 pts (10.3%) died; 89 (8.4%) developed heart failure
- Outcomes of pts with resolving anemia similar to those of pts without anemia
- Marked increase in mortality and heart failure in pts with persistent and new-onset anemia
