Todd H - Pain is a common reason for emergency department visits and patients with histories of substance abuse are overrepresented in the emergency department population. The management of patients on methadone maintenance therapy who present to the emergency department with acute pain resulting from illness or injury is particularly complex. This case study examines the care of a patient receiving methadone maintenance therapy who presented to the emergency department after suffering a tibial plateau fracture. In this case study the authors identify potential pitfalls in therapy and offer recommendations for pain treatment that should result in optimal outcomes.
