This review presents an interesting discussion on experimental observations regarding the nature of the odontogenic keratocyst (OKC). The change of category of OKC in the last WHO revision came as a surprise to many and is still being questioned. This paper reviews in depth several molecular aspects of the condition including PTCH1 mutations and involvement of the Hedgehog signaling pathway, that may not only be helpful in defining OKC but also hint to potential molecular treatment approaches for this troublesome entity.
