Influences on dentists' decisions to refer paediatric patients to dental hygienists and therapists for fissure sealants: a qualitative approach

| Continued changes in dentistry and oral health status have prompted a need to review the roles of dental auxiliaries in delivering quality care cost-effectively (Department of Health, 2000). Therefore, there were two reasons for undertaking this work. First of all, if we are to increase the use of skill-mix it is important to know how dental teams work together. As prevention is an important part of the role of hygienists and therapists we decided to study the way dentists refer patients to them for fissure sealants as a case-study in team working. The data indicate a number of factors that might influence referral decisions and these factors point to ways in which the use of skill mix might be increased. For example, dentists may need training in the possible roles of different groups of workers. Perhaps more contentiously, the nature in which dentists are paid might be changed to encourage greater delegation of care. Another important reason for the research is to help study the effectiveness of fissure sealants placed by different dental workers. An earlier systematic review (Galloway et al. 2003) concluded that the overall standard of research relating to the effectiveness of work undertaken by dental care professionals (DCPs) (including fissure sealants) was poor. The majority of the research was out of date, having been conducted during the 1970s. There is therefore a clear need for high quality research in this area to be undertaken in the UK. However, comparisons of the relative effectiveness of sealants placed by dentists and DCPs must also consider any factors that might confound the relationship between operator type and sealant success. The present study has provided this essential information. The factors identified in this study can therefore be taken into account when designing studies to compare the relative effectiveness of both operator types. |
