Clinical Pearls in Anesthesiology

Highlights in Anesthesiology

Dopamine is associated with increased mortality in septic shock

Based on a meta-analysis of 11 trials involving 2,768 patients with septic shock, treatment with dopamine was associated with a greater risk of 28-day mortality (RR=1.12-1.23) and arrhythmias (RR=2.34) than norepinephrine.

RBC transfusion decreased mortality in sepsis

As published in Critical Care Medicine, RBC transfusion decreases the risk of mortality in patients with sepsis and septic shock. The study involved 1054 patients in 22 ICUs and SICUs at 12 teaching hospitals in Korea. Of the 1054 patients, 407 received RBC transfusions; the mean pre-transfusion hemoglobin level was 7.7 g/dL. The transfused patients had a lower risk of 7-day (HR = 0.42), 28-day (HR = 0.43), and in-hospital mortality rates (HR = 0.51) than non-transfused patients.

Statin use decreases pneumonia-associated mortality

As published in Critical Care, current statin use was associated with a decreased hospitalization rate and a decreased 30-day mortality rate in patients with pneumonia. The study group consisted of 70,953 patients with pneumonia who were hospitalized for the first time with 10 age- and gender-matched control patients for each pneumonia patient. The OR for pneumonia was 0.80 amongst current statin users compared to non-users; previous statin use had no effect on the risk of pneumonia. The 30-day mortality rates in patients who were statin users and non-users were 11.3% and 15.1%, respectively (HR=0.73).
