Ingber MS et al. – Pain experienced during transrectal biopsy of the prostate is mild and is not significantly lowered with periprostatic nerve block. Pain from injection itself is similar to pain from core biopsies. Pain from transrectal ultrasound–guided biopsy of the prostate is well tolerated with no anesthesia.

The utilization of local anesthetic during prostate biopsy has been highly controversial over the years. Most of the literature has focused on periprostatic injection of lidocaine, although other modalities have been described. This study was created at our institution after two of our urologists debated their beliefs. One urologist had always used periprostatic injection of lidocaine prior to biopsy and the other never used any local anesthesia. Our results from the study confirm that pain during prostate biopsy is minimal, and does not require anesthesia. Pain from the lidocaine injection itself produced pain that was as severe as the actual biopsy itself. Most patients had very little pain at the end of the biopsy procedure. We look forward to other studies which can confirm our findings.
