We report a case of a 47-year-old male undergoing endoscopic removal of a third ventricular colloid cyst. After uneventful surgery, the patient remained drowsy and was transferred to the intensive care unit for supportive care. In the postoperative period, the patient developed hydrocephalus due to clot in the region of the cyst. A posterior fossa haematoma and further neurological deterioration complicated external ventricular drainage, presumably due to sudden intracranial hypotension
