Bosteels J et al. – Scarce evidence on the effectiveness of hysteroscopic surgery in subfertile women with polyps, fibroids, septate uterus or intrauterine adhesions indicates a potential benefit. More randomized controlled trials are needed before widespread use of hysteroscopic surgery in the general subfertile population can be justified.

The current systematic review critically apraises the effectiveness of hysteroscopic procedures, both diagnostic and therapeutic, in subfertile women without other gynaecological symptoms.Two problems are detected. First, there are only few data provided by randomized trials. Second, the clinical situation described in the few available trials differ from daily life in that practicing clinicians are frequently confronted with patients who have besides their subfertility isssue other gynaecological complaints, such as bleeding or pain due to fibroids, recurrent miscarriage due to septa or past history of obstetric complications. It is imperative to investigate the human implantation factor in reproductive physiology by promoting RCTs while taking into account these additional factors.
