Subramanian U et al. – Results extend prior observations that diabetic pts with congestive heart failure (CHF) may not accrue the same mortality benefit from cardioselective beta-adrenoceptor antagonists (CSB) therapy as non-diabetic pts, pending further prospective study.
Methods- Study of whether diabetic pts with CHF accrue the same mortality benefit from CSB therapy as non-diabetic pts
- Randomized controlled trial October 1999-November 2000 for consecutive CHF pts
- Prospective follow-up for 5 yrs
- Disease severity and CHF-specific functional status from pts at baseline
- Medical record review for data on co-morbidities, medications, and mortality
- Propensity-score analysis to balance co-variates due to observational nature of CSB use, for post hoc analysis
- Multivariate Cox proportional hazards model to compare survival between diabetic and non-diabetic pts stratified by CSB therapy or no CSB therapy
- Of 412 evaluable pts, diabetes for 222 pts (54%); CSB use for 212 pts (51%)
- At 5-yr follow-up, mortality for 186 pts (45%)
- In multivariate analysis, using propensity scores to balance co-variates, CSB therapy was an independent predictor of survival non-diabetic pts
